Tilda Swinton as The Goblin Princess, trying to cement her claim as queen in the wake of her father's passing. She hatches a plan and kidnaps a mentally retarded young woman named Clarity, played by Heather Matarazzo, who's nephew Carter, played by Anton Yelchin has to spend his days taking care of her, and cleaning her up when she makes a mess, because his grand parents can't afford a professional caretaker.

Carter resents his parents for dying in a car wreck and leaving him to be raised by his grand parents, who he resents for making him an indentured servant who's only value is in freeing them up from having to take care of their daughter in the afternoons when he gets home from school. Most of all he resents Clarity for being the reason he has no social life, and couldn't even bring friends home from school (if he had any).

The scenes with Carter navigating the Labyrinth are interspersed with scenes where Clarity is hanging out with the Goblin Princess and she starts to live up to her name. The magical realm is somehow healing Clarity's mind, and for the first time in her life, she can speak her thoughts and understand the words written on pages. When Carter arrives to "save" her, she refuses rescue in favor of staying in a world where she's no longer feeble. Carter viciously lashes out at her verbally (maybe even slaps her to try to bring her to her senses), because he knows that if she doesn't come back with him, the grand parents are going to blame him and his life will be even worse.

The back half of the movie is about Clarity fleeing from her deranged nephew and seeking solace with the Goblin Princess, who's starting to fall in love with her. During the climax when Princess Sareth (no, she's not the daughter of Jareth and Sarah, it turns out that he named her after him and Sarah because he liked the sound of the names combined) sacrifices herself to send Carter back to the real world, the magic of the Labyrinth heals her because she's demonstrated the nobility that is becoming of a Queen. The Goblin Queen and her lover are betrothed and have a happy ending.

Meanwhile Carter ends up back home, with the family convinced that the pressure of caring for his sister and not being able to find her when she runs away have caused a delusional break with reality. He's given sessions with a therapist and plays along so that he can be given a clean slate. It also turns out that the reason Gramma and Gramps have been having him take care of Clarity all these years, is so they could set aside the monthly wages they'd have otherwise had to pay a professional, and they've built up a college fund for him. Now he gets to feel like a shitty grand son, and a shitty nephew to boot.

Damn... Arkay, thank you for inspiring a notion that I might rework into a stand alone story someday.

I actually thought Tilda when I thought Goblin Queen.  But I also had the thought that it might be too close, because she does favor Bowie.  But going with a daughter angle makes it a reason for her to resemble him, beyond just trying to mimic him.
